BACKGROUND: Pneumococcal carriage studies provide a baseline for measuring the impact of pneumococcal conjugate vaccines (PCVs). The advent of conjugate vaccines has led to reductions in vaccine serotypes (VTs) in pneumococcal carriage. However, increasing non-vaccine serotypes (NVTs) remain a significant concern, necessitating continued surveillance of serotypes in the 13-valent PCV vaccine (PCV13) era.OBJECTIVES: To investigate pneumococcal carriage, serotype distribution and risk factors for pneumococcal colonisation among children presenting for routine immunisation at two clinics in Gauteng Province, South Africa (SA), 10 years after PCV introduction into the SA Expanded Programme on Immunisation (EPI-SA).METHODS: Nasopharyngeal swabs were collected from 322 healthy children aged between 6 weeks and 5 years at two clinic centres in 2014 and 2016. Demographic data, risk factors for colonisation and vaccination details were recorded. The pneumococcal isolates were serotyped and tested for antimicrobial susceptibility.RESULTS: Pneumococci were isolated from 138/316 healthy children (43.7%) presenting for routine immunisation at two clinics. The median age was 8.3 months and the age range 1.4 months - 5 years. Carriage varied across the age groups: 6-14 weeks 35.5%, 9 months 27.5%, 18 months 21.7%, and 5 years 15.2%. Risk factors significantly associated with pneumococcal colonisation included young age (9 -18 months (odds ratio OR 3.5; 95% confidence interval (CI) 1.9 - 5.9), type of dwelling (single room (OR 8.1; 95% CI 1.3 - 52.3) or informal dwelling (OR 2.4; 95% CI 1.2 - 4.5)) and Haemophilus influenzae carriage (OR 5.6; 95% CI 0.6 - 2.5). Of the 26 serotypes detected, 19F (10/121; 8.3%) was the most frequent. The most frequent NVTs were 23B (16/121; 13.2%), 15B/C (14/121; 11.6%) and 35B (11/121; 8.2%). Children aged 9 months carried the highest proportion of NVTs (33/101; 32.7%). Penicillin non-susceptibility was observed in 20 NVT isolates (20/36; 55.6%) and 2 VT isolates (2/36; 5.6%).CONCLUSIONS: The pneumococcal carriage prevalence described in our study varied across the age groups and was lower compared with other African studies that looked at pneumococcal carriage post PCV. The study gave insight into the common NVTs encountered at two immunisation clinics in Gauteng. Given that pneumococcal carriage precedes disease, common colonisers such as 15B/C and 35B may be sufficiently prevalent in carriage for expansion to result in significant disease replacement.

BACKGROUND: Although immunisation services are available to all children in South Africa (SA), many children miss or have delays in receiving vaccines. There are limited data on factors associated with missed or delayed vaccination in children in this setting.OBJECTIVES: To assess vaccination coverage and factors associated with missed and delayed diphtheria-tetanus-pertussis vaccine third dose (DTP3) vaccination in children aged 12 - 59 months in two SA communities.METHODS: We used data from household-level healthcare utilisation surveys conducted in Soweto in 2012 and in Pietermaritzburg in 2013. Information on vaccination status was recorded from the Road to Health cards or vaccination history from clinics for children aged <5 years. Factors associated with missed or delayed DTP3 vaccination were assessed using unconditional logistic regression.RESULTS: Of a total of 847 eligible children aged 12 - 59 months, 716 had available vaccination information. Overall DTP3 vaccination coverage was high for both sites: 90.6% in Pietermaritzburg and 93.9% in Soweto. However, 32.6% and 25.2% of DTP3 vaccinations were delayed (received after 18 weeks of age) in Pietermaritzburg and Soweto, respectively. The median delay for DTP3 vaccinations was 4.7 weeks (interquartile range 1.7 - 23.0). Factors associated with delayed DTP3 vaccination included being born in 2010 (adjusted odds ratio (aOR) 3.0, 95% confidence interval (CI) 1.4 - 6.3) or 2011 (aOR 2.7, 95% CI 1.3 - 5.7) compared with being born in 2008, probably due to vaccine shortages; a low level of education of the primary caregiver, with children whose caregivers had completed secondary education having lower odds of delayed vaccination (aOR 0.5, 95% CI 0.3 - 0.9) than children whose caregivers only had primary education; and maternal HIV status, with unknown status (aOR 3.5, 95% CI 1.6 - 7.6) associated with higher odds of delay than positive status. Factors associated with missed DTP3 vaccination (not vaccinated by 12 months of age) included two or more children aged <5 years in a household (aOR 2.4, 95% CI 1.2 - 4.9) compared with one child, and household monthly income <ZAR500 (aOR 3.4, 95% CI 1.1 - 11.4) compared with >ZAR2 000.CONCLUSIONS: Despite high overall DTP3 coverage observed in two communities, many vaccinations were delayed. Vulnerable groups identified in this study should be targeted with improved vaccination services to enhance uptake and timeliness of vaccination.

OBJECTIVE: To describe findings from an external quality assessment programme involving laboratories in Africa that routinely investigate epidemic-prone diseases. METHODS: Beginning in 2002, the Regional Office for Africa of the World Health Organization (WHO) invited national public health laboratories and related facilities in Africa to participate in the programme. Three surveys comprising specimens and questionnaires associated with bacterial enteric diseases, bacterial meningitis, plague, tuberculosis and malaria were sent annually to test participants' diagnostic proficiency. Identical surveys were sent to referee laboratories for quality control. Materials were prepared, packaged and shipped in accordance with standard protocols. Findings and reports were due within 30 days. Key methodological decisions and test results were categorized as acceptable or unacceptable on the basis of consensus feedback from referees, using established grading schemes. FINDINGS: Between 2002 and 2009, participation increased from 30 to 48 Member States of the WHO and from 39 to 78 laboratories. Each survey was returned by 64-93% of participants. Mean turnaround time was 25.9 days. For bacterial enteric diseases and meningitis components, bacterial identification was acceptable in 65% and 69% of challenges, respectively, but serotyping and antibiotic susceptibility testing and reporting were frequently unacceptable. Microscopy was acceptable for 73% of plague challenges. Tuberculosis microscopy was satisfactorily performed, with 87% of responses receiving acceptable scores. In the malaria component, 82% of responses received acceptable scores for species identification but only 51% of parasite quantitation scores were acceptable. CONCLUSION: The external quality assessment programme consistently identified certain functional deficiencies requiring strengthening that were present in African public health microbiology laboratories.

OBJETIVO: Describir los resultados de un programa de evaluación externa de la calidad de los laboratorios africanos que investigan habitualmente las enfermedades con tendencia a provocar epidemias. MÉTODOS: En el año 2002, la Oficina Regional para África de la Organización Mundial de la Salud (OMS) invitó a los laboratorios nacionales de la sanidad pública así como a los centros relacionados de África a participar en el programa. Con el fin de comprobar la capacidad de diagnóstico de los participantes, se les enviaron anualmente tres encuestas con muestras y cuestionarios relacionados con enfermedades enterobacterianas, meningitis bacteriana, peste, tuberculosis y malaria. Con el fin de realizar un control de calidad, se enviaron las mismas encuestas a los laboratorios evaluadores. Los materiales se prepararon, empaquetaron y enviaron de acuerdo con los protocolos estándar. Los resultados y las encuestas debían estar listos en un plazo de 30 días. Las principales decisiones metodológicas y los resultados de las pruebas se clasificaron como aceptables o inaceptables en base a la respuesta consensuada de los evaluadores por medio de los programas de calificación establecidos. RESULTADOS: La participación aumentó desde 30 hasta 48 Estados Miembros de la OMS y desde 39 hasta 78 laboratorios entre los años 2002 y 2009. Entre un 64 y un 93% de los participantes remitieron cada una de las encuestas y el plazo medio de entrega fue de 25,9 días. En los casos de enfermedades enterobacterianas y de meningitis, la identificación bacteriana fue aceptable en un 65% y un 69% de los casos, respectivamente, si bien las pruebas y los resultados de serotipado y de sensibilidad a los antibióticos fueron, con frecuencia, inaceptables. La microscopía resultó aceptable en el 73% de las pruebas de peste. La microscopía de tuberculosis se realizó de manera satisfactoria: un 87% de las respuestas obtuvo resultados aceptables. En el componente de la malaria, un 82% de las respuestas obtuvieron puntuaciones aceptables en cuanto a la identificación de la especie, pero solo el 51% de las puntuaciones para la cuantificación de parásitos resultó aceptable. OBJECTIF: Décrire les résultats d'un programme d'évaluation externe de la qualité impliquant des laboratoires africains étudiant de matière systématique les maladies à potentiel épidémique. MÉTHODES: Dès 2002, le Bureau régional pour l'Afrique de l'Organisation mondiale de la Santé (OMS) a invité les laboratoires de santé publique nationaux et des installations connexes en Afrique à participer au programme. Trois études comprenant des échantillons et des questionnaires associés aux maladies entériques bactériennes, à la méningite bactérienne, à la peste, à la tuberculose et au paludisme ont été envoyés chaque année pour tester la qualité du diagnostic des participants. Des études identiques ont été envoyées à des laboratoires arbitres pour le contrôle de la qualité. Les matériaux ont été préparés, emballés et expédiés conformément aux protocoles standard. Les résultats et les rapports devaient parvenir dans les 30 jours. Les principales décisions méthodologiques et les résultats des tests ont été qualifiés d'acceptables ou d'inacceptables, sur la base des commentaires consensuels des arbitres, utilisant des schémas de notation établis. OBJECTIVE: To analyse trends in reported invasive Haemophilus influenzae disease in South Africa within the first five years of introduction of conjugate Haemophilus influenzae type b (Hib) vaccine in the routine child immunization schedule. METHODS: We used national laboratory-based surveillance data to identify cases of invasive H. influenzae disease between July 1999 and June 2004, and submitted isolates for serotyping and antimicrobial susceptibility testing. FINDINGS: The absolute number of Hib cases (reported to the national surveillance system) among children below one year of age decreased by 65%, from 55 cases in 1999-2000 to 19 cases in 2003-04. Enhanced surveillance initiated in 2003, identified human immunodeficiency virus (HIV)-infection and incomplete vaccination as contributing factors for Hib transmission. The total number of laboratory-confirmed cases of H. influenzae remained unchanged because non-type b disease was being increasingly reported to the surveillance system concomitant with system enhancements. Children with non-typable disease were more likely to be HIV-positive (32 of 34, 94%) than children with Hib disease (10 of 14, 71%), P = 0.051. Recent Hib isolates were more likely to be multidrug resistant (2% in 1999-2000 versus 19% in 2003-04, P = 0.001). CONCLUSION: Data from a newly established national laboratory-based surveillance system showed a decrease in Hib disease burden among South African children following conjugate vaccine introduction and identified cases of non-typable disease associated with HIV infection.

OBJETIVO: Analizar las tendencias de los casos notificados de enfermedad invasiva por Haemophilus influenzae en Sudáfrica durante los cinco primeros años de utilización de la vacuna conjugada contra Haemophilus influenzae tipo b (anti-Hib) en el calendario de vacunación infantil sistemática. MÉTODOS: Usamos los datos de vigilancia obtenidos por el laboratorio nacional para identificar los casos de enfermedad invasiva por H. influenzae registrados entre julio de 1999 y junio de 2004, así como las cepas aisladas enviadas para serotipificación y análisis de la sensibilidad a los antimicrobianos. RESULTADOS: La cifra absoluta de casos de infección por Hib (notificados al sistema de vigilancia nacional) entre los niños menores de un año se redujo en un 65%, de 55 casos en 1999- 2000 a 19 casos en 2003-2004. La vigilancia mejorada iniciada en 2003 identificó la infección por el virus de la inmunodeficiencia humana (VIH) y la vacunación incompleta como factores contribuyentes a la transmisión de Hib. El número total de casos de infección por H. influenzae confirmados en laboratorio se mantuvo inalterado debido a que la enfermedad de tipo no b se notificó con creciente frecuencia al sistema de vigilancia como consecuencia de la mejora del sistema. Los niños con enfermedad no tipificable tenían más probabilidades de ser VIH-positivos (32 de 34; 94%) que los niños con enfermedad por Hib (10 de 14; 71%), P = 0,051, y los aislados recientes de Hib tenían más probabilidades de ser multirresistentes (2% en 1999-2000, frente al 19% de 2003-2004, P = 0,001). CONCLUSIONES: Los datos de un nuevo sistema nacional de vigilancia basado en el laboratorio mostraron una disminución de la carga de morbilidad por Hib entre los niños sudafricanos tras la introducción de vacunas conjugadas e identificaron casos de enfermedad no tipificable asociada a la infección por VIH.

OBJECTIF: Analyser les tendances observées dans les infections invasives à Haemophilus influenzae en Afrique du Sud pendant les cinq premières années suivant l'introduction du vaccin conjugué anti-Haemophilus influenzae type b (Hib) dans le calendrier de vaccination systématique de l'enfant. MÉTHODES: On a utilisé les données de la surveillance nationale au laboratoire pour définir les cas d'infection invasive à H. influenzae entre juillet 1999 et juin 2004 et soumis des isolements au sérotypage et à des épreuves de sensibilité aux antimicrobiens. RÉSULTATS: Le nombre absolu de cas d'infection à Hib (notifiés au système de surveillance national) chez l'enfant de moins d'un an a diminué de 65 %, passant de 55 en 1999-2000 à 19 en 2003-2004. Le renforcement de la surveillance commencé en 2003 a permis de constater que l'infection par le virus de l'immunodéficience humaine (VIH) et la vaccination incomplète constituaient des facteurs contribuant à la transmission du Hib. Le nombre total de cas d'infection à H. influenzae confirmés au laboratoire est resté inchangé, les infections n'appartenant pas au type b étant de plus en plus notifiées au système de surveillance à mesure que celui-ci devenait plus performant. Les enfants touchés par une infection à Haemophilus de type impossible à déterminer étaient plus souvent VIH-positifs (32 sur 34, 94 %) que les enfants infectés par Hib (10 sur 14, 71 %, p = 0,051). On a également observé une probabilité plus grande de multirésistance dans les isolements récents de Hib (2 % en 1999-2000 contre 19 % en 2003-2004, p = 0,001). CONCLUSION: Les données issues d'un système national de surveillance au laboratoire récemment mis en place ont fait apparaître une diminution de la charge de morbidité due à Hib chez les enfants sud-africains à la suite de l'introduction du vaccin conjugué et permis d'identifier des infections à Haemophilus dont le type était impossible à déterminer, associées à l'infection par le VIH.
